IMMUNIZATION RECORDS

Children must be properly immunized according to the laws of the State of New Mexico. A child will not be allowed to enroll or stay in school without the proper immunization(s). The state law also requires that all students participating in extra-curricular activities must have a physical or he/she cannot participate in sport activities.

All students must have the following schedule of immunizations on file at school before they can attend:

  • A child entering OOCS for the first time must be vaccinated against: Polio, Measles, (1 day) or
    Red Measles, Rubella (German or 3 day Measles), DPT, Mumps, and any other immunizations
    as required by the NMHED.

If a parent is a conscientious objector to the immunization of their son/daughter, the school must have a certified verifying that fact. The certificate may be obtained at IHS or at the Santa Clara Clinic. This certificate must be renewed every nine months. Another address to request exemption from immunizations is:
